Anticoagulants such as warfarin (Coumadin?) or dabigatran (Pradaxa?) are important therapies for patients at an increased risk for blood clots. Therapy with warfarin must be closely regulated to be certain that you are receiving the right dose.
If you take a blood thinner, your doctor can refer you to Anticoagulation Services. Our pharmacists specialize in anticoagulation therapy and will help manage your care.
On your first visit, we will help you understand your anticoagulation therapy and give you an educational packet and pillbox. For follow-up visits you will receive one-on-one consultations with a pharmacist as well as written instructions for warfarin dosing. Our staff will perform blood tests and ask about the medications you are taking (prescription, non-prescription and herbal), foods you are eating and other health issues.
New patients can expect their first visit to be approximately 40 minutes. Follow-up visits are 10 to 20 minutes. A note from each visit will be forwarded to your physician.
To be enrolled in the anticoagulation clinic, you must:
- Be able to travel to and from the clinic for appointments.
- Have a local physician in case of urgent health status changes (Northern Idaho/Spokane area).
- Be referred by the physician who prescribes your blood thinners.
